Noticing hearing issues? A diagnostic, comprehensive hearing test to determine function of outer, middle and inner ear system will be performed. Medicare requires medical necessity and a physician referral to be billed. It is the responsibility of the patient to request the physician order that must be received prior to the appointment. If we are out of network with your insurance, we will bill as a courtesy. Full payment is due at time of service. Pediatric Hearing Tests, ages 4 and older, available upon request (please call or email). We are unable to see pediatrics under age 4. Estimated adult price quote: $85-115.

Ear Wax Removal
This appointment is for individuals that only need professional ear wax removal or to remove foreign bodies from the ear canal. This appointment includes video otoscopy so you can also look inside your ears and professional removal of wax. Estimated price quote: $30-60 per ear depending on level of difficulty to remove. *Please note, as this appointment is being held for you, the base cost of the office visit ($49) will be due at time of service even if earwax build-up is not found.

Earmold Impressions
This appointment is for individuals needing earmold impressions for in-ear monitors, swim plugs, custom headphones, custom earplugs, hearing aid molds, etc. A 30-minute visit for highly skilled outer ear impressions which includes video otoscopy, earwax removal (if necessary to get an excellent impression), and one or two impressions. Estimated price quote: $95. *Custom ear piece prices vary depending on style and model.

New Patient - Hearing Aid Check
For patients needing ongoing care of their devices that are adopting us as their new audiologist. Additional time is allotted to get to know you, save intake hearing aid settings, enter in demographic hearing aid information, and determine need for care and maintenance. Includes visual inspection of the ears and any wax removal, hearing aid cleaning and dehumidifying, small parts changes, warranty checks, and listening checks. This appointment has a fee of $95. There is an additional charge for programming adjustments, firmware updates, custom products, or hearing aid parts are required that are out-of-warranty. Service plans are available at $365 per year to include all office services (excludes manufacturer repairs, earmolds, accessories, and loss & damage fees).

New Patient - Hearing Aid Repair
Is your hearing aid(s) broken, dead, or not working properly? Are you having some trouble with your hearing aids? We’ll take a look at everything and determine what is required to ensure you’re back in business! The minimum charge for a 30 minute appointment is $95 which includes a thorough cleaning and assessment of the device(s). If hearing aid diagnostics are required to ensure the hearing aids are functioning at their manufacturer specifications, this is $45 per device. If parts are required, there may be an additional cost if your devices are out of warranty.

Tinnitus Evaluation
This appointment is for individuals with chronic tinnitus- ringing, buzzing, humming, etc., in the ears/brain. There is hope! During this appointment we will complete a diagnostic hearing exam and additional tinnitus measurements (as needed). We will be able to discuss your needs and expectations and build the proper management plan for you. Medicare requires medical necessity and a physician referral to be billed. It is the responsibility of the patient to request the physician order that must be received prior to the appointment. If we are out of network with your insurance, we will bill as a courtesy. Full payment is due at time of service. The estimated cost is $480 including your education and counseling, diagnostic tests (hearing and tinnitus assessment), initial treatment plan, and 1-30 minute follow up visit via phone call or video conference 4 weeks after this evaluation. If other services are required, there are additional costs.

Existing Patient - Hearing Aid Wellness Check Up
Critical check-up appointment are highly recommended for proper maintenance of hearing aids. These typically occur every 6 months, more often for some. Includes visual inspection of the ears and any wax removal, enhanced hearing aid cleaning and dehumidifying, small parts changes, warranty checks, listening checks, real ear measurements, basic programming adjustments, and firmware updates. No cost for patients with a service plan. Estimated price for patients without a service plan is $145. There is an additional charge if additional services such as advanced hearing aid programming, electroacoustic analysis of hearing aids, or replacement of broken parts are required.

Veteran Disability Claim Private Examination
A diagnostic, comprehensive hearing test to determine function of outer, middle and inner ear system. Veteran's Administration testing protocol will be used. Veteran should provide records for review, such as: DD214, entrance exam, exit exam, and any other pertinent or related forms if available. Written report and Nexus letter (if needed) will be provided within 48-72 hours (business days) after exam. Patients with Medicare, or Tricare plans will require an order/referral/prior authorization from a Primary Care Physician if they wish bill to be submitted to insurance*.
