Earmold Impressions (in-ear monitors, custom earplugs, etc.)
This appointment is for individuals needing earmold impressions for in-ear monitors, swim plugs, custom headphones, custom earplugs, etc.
$105 for 30 minute visit for highly skilled outer ear impressions which includes video otoscopy, earwax removal (if necessary to get an excellent impression), and one or two impressions.
Custom product prices vary depending on style and model.

This appointment is for individuals that only need professional ear wax removal. A 30 minute appointment is $110 and includes video otoscopy so you can also look inside your ears.
*Please note, as this appointment is being held for you, this is the cost of the office visit which will be due at time of service even if earwax build-up is not found.

Functional Hearing Assessment
This appointment time is used for additional testing and consultation to develop a personalization Plan of Care. You must already have a copy of a diagnostic hearing exam performed by an audiologist within the past 6 months. Additional diagnostic testing may be required based on the thoroughness of the exam and report presented to us. The expected cost for these services is $99, additional fees may be incurred based on the need for additional diagnostics or services provided.

Hearing Aid Issue
Are you having some trouble with your hearing aids? We’ll take a look at everything and determine what is required to ensure you’re back in business!
No charge for appointment if you have a service plan with us. Without a service plan the price varies depending on services required. The minimum charge for a 30 minute appointment is $110 which includes a thorough cleaning and assessment of the device(s). If hearing aid diagnostics are required to ensure the hearing aids are functioning at their manufacturer specifications, this is $47.50 per device.
If parts are required, there may be an additional cost if your devices are out of warranty.

Hearing Wellness Check
Critical check-up appointments for proper maintenance of hearing aids. Typically occur every 4-6 months, more often for some. Includes visual inspection of the ears and any wax removal, hearing aid cleaning and dehumidifying, small parts changes, warranty checks, listening checks, basic programming adjustments, and firmware updates.
No cost for patients with a service plan. $110 for those without a service plan. There is an additional charge if advanced hearing aid programming, electroacoustic analysis of hearing aids, or additional services such as real-ear measurements are required.

Hyperacusis Evaluation and Education
This appointment is for individuals with hypersensitivity to sound.

Medicare requires medical necessity and a physician referral to be billed. It is the responsibility of the patient to request the physician order that must be received prior to the appointment. Education and treatment services will not be covered by Medicare and are the financial responsibility of the patient. Full payment is due at time of service.
The cost is $264-$390 including your education and counseling, diagnostic tests, and initial treatment plan. If other services or devices are required, there are additional costs.

This appointment is for individuals with diagnosed or suspected misophonia.

Education and treatment services will not be covered by Medicare and are the financial responsibility of the patient. Full payment is due at time of service.

New Patient Evaluation & Functional Hearing Assessment
This appointment is for individuals requiring a Diagnostic Hearing Evaluation in addition to a Functional Hearing Assessment (FHA). The evaluation includes visual inspection of the ear canals, air conduction, bone conduction, and speech testing in quiet and in noise as well as a cognitive screening, inventories, and other determinations that are appropriate to create a Plan of Care for your individual needs. The service cost is $99 for the diagnostics and $99 for the FHA.
Additional services such as earwax removal, middle ear measurements (tympanometry), or acoustic reflex testing will performed as needed with additional fees for those services.
Medicare requires medical necessity and a physician referral to be billed. It is the responsibility of the patient to request the physician order that must be received prior to the appointment. We are out of network with all other insurances and will bill as a courtesy. Full payment is due at time of service.

Hearing Aid Check-Up for New Patients with Hearing Aids
For patients needing ongoing care of their devices that are adopting us as their new audiologist.
Additional time is allotted to get to know you, save intake hearing aid settings, enter in demographic hearing aid information, and determine need for care and maintenance.

This appointment is for individuals with chronic tinnitus- ringing, buzzing, humming, etc., in the ears/brain.

The cost is $264-$390 including your education and counseling, diagnostic tests (hearing and tinnitus assessment), and initial treatment plan. If other services are required, there are additional costs.

Tinnitus Management Consultation
This appointment is for individuals with bothersome tinnitus that have already had a hearing and tinnitus evaluation. During this appointment, we demonstrate and discuss tinnitus management options including sound enrichment therapy appropriate for the individual. At the end of this appointment, the patient will have a thorough understanding of their options moving forward and continue their plan of care.
This appointment is typically 1.5 hours in length and is $285. This is not covered by medical insurance and payment is due at time of service.
Additional follow-up visits will be scheduled as needed.
