Auditory Processing Disorder Screening
A diagnostic, comprehensive evaluation to determine function of outer, middle and inner ear system. Patients with Medicare, or HMO plans will require an order/referral from a Primary Care Physician if they wish bill to be submitted to insurance. Screenings are to determine if further testing is necessary. Patients may be referred for further testing depending on results. Comprehensive Hearing Test
A diagnostic, comprehensive hearing test to determine function of outer, middle and inner ear system. If patients are current hearing aid users, would recommend Hearing Test + Hearing Aid Evaluation appointment instead. Pediatric Hearing Tests available upon request (please call or email).

Hearing Test + Hearing Aid Evaluation
A diagnostic, comprehensive hearing test to determine function of outer, middle and inner ear system, plus clean and check assessment of current hearing aid technology. Patients with Medicare, or HMO plans will require an order/referral from a Primary Care Physician if they wish for bill to be submitted to insurance. Not all charges may be eligible for insurance coverage.

Ear wax removal
An appointment for a new or established patient, to remove ear wax or foreign bodies from the ear canal.
*Please note, as this appointment is being held for you, the base cost of the office visit will be due at time of service even if earwax build-up is not found.

This appointment is for individuals that would like to meet with our doctors to discuss their audiology needs and treatment options.

Earmold Impressions
An appointment for a new or established patient, to have impressions of the ear canals made for custom earmolds, swimplugs, in-ear monitors, hearing protection, musician's plugs, etc. Costs for molds or custom ear products may vary depending on manufacturer.

Hearing Aid Clean and Check
An appointment for an established patient, to clean and check current hearing aids. Other charges may apply (out-of-warranty repairs, electroacoustic analysis, Real-Ear Measurement, etc).

Hearing Screening
A pass or fail hearing screening. This assessment is for patients of all ages (pediatric to geriatric), to determine if hearing loss may be present, and determine if a more comprehensive diagnostic evaluation is needed. This screening will not determine hearing aid or cochlear implant candidacy, or rule out minimal or mild hearing losses.

Tinnitus Evaluation
This appointment takes two hours, during which time a comprehensive diagnostic audio evaluation will be performed. Other tests included during this exam will be: high frequency audiometry, tympanometry, reflexes, reflex decay, diagnostic otoacoustic emissions and a tinnitus assessment. Test results will be explained, and a treatment plan will be discussed. This evaluation is not eligible for insurance submission by our office, but patients may file at their discretion.

Veteran Disability Claim
A diagnostic, comprehensive hearing test to determine function of outer, middle and inner ear system. Veteran's Administration testing protocol will be used. Veteran should provide records for review, such as: DD214, entrance exam, exit exam, and any other pertinent or related forms if available. Written report and Nexus letter (if needed) will be provided within 48-72 hours (business days) after exam. *These exams are not eligible for insurance submissions*.
