Patients who are interested in a hearing evaluation, to discuss candidacy and pricing. Appointment will include all or some of the following services: Intake questionnaires, cognitive screening, comprehensive hearing evaluation, speech in noise testing, review of test results, a communication needs assessment, and receive a formal recommendation with pricing. Insurance verification will be performed if member ID provided ahead of time.
