Established patient - needs new hearing test & wants current hearing aids cleaned
I'm already established with the clinic and would like a new hearing test in the sound booth and I would like my current hearing aids cleaned and checked. I understand my insurance may require medical order for all services (true for all HMO plans) and Active Hearing Health may not be able to bill my insurance if I do not contact my primary care physician to request they send a medical order.

Established patient - test & consult to explore hearing aids
I'm established at the clinic. I'd like to update my hearing test and learn about current hearing aid options for me. I understand Active Hearing Health may not be able to bill my insurance if I do not have my primary care physician send over a medical order for a hearing test prior to my appointment (true for all HMO plans). I understand that my appointment time will be best served if I can provide copies of my insurance card in advance so that Active Hearing Health can complete a verification of hearing aid benefits and then use that information to calculate my expected out of pocket pricing on new hearing aids after insurance.

New Patient - Hearing Test Only
I'm new to your office and I'd like a hearing test in the soundbooth. I only want the test at this time. I do not want to talk about hearing aids at this time. I understand Active Hearing Health may not be able to bill my insurance if I do not contact my primary care physician to have them send over a medical order for the test in advance of my appointment (true for all HMO plans).

Aid problems - Bluetooth connectivity
I need my aid(s) checked. They are not staying connected to my phone. I understand that no insurance plan covers connectivity issues with phones as insurance does not consider Bluetooth connectivity to be a medically necessary service.

Clean & Check of Hearing Aids
I would like my devices to be thoroughly cleaned and checked. I'm not having any advanced problems. I understand if my insurance plan is an HMO plan, Active Hearing Health cannot bill my insurance unless I havve a medical order for services from my primary care provider in place prior to my appointment.
