Annual Hearing Test
Annual hearing test and wellness check. This appointment type is most appropriate for patients that have had a hearing test within the past 12-18 months and are wanting an updated test. Please let our staff know if you are having any issues with your hearing devices that will need to be addressed during the visit. Annual testing is $100.

Hearing Evaluation (Existing Hearing Device Patient)
This visit is most appropriate for patients that have not had a hearing test in over 3 years. Visit cost is $200 or can be billed to insurance.

Hearing Test (New Patient)
This visit is most appropriate for patients that have not had a hearing test and/or do not currently wear hearing devices. Extra time is allotted to discuss treatment options and to do a hearing device demonstration, if appropriate. Visit cost is $200 or can be billed to insurance.

Hearing Device Demonstration
Appointment is most appropriate for patients that have had a hearing test in the past 6 months and would like to discuss treatment options at this time. Time is allotted for hearing device demonstration.

Hearing Test (testing only)
This appointment is most appropriate for patients needing a hearing test for school, work, physical assessment, or as requested by a physician as part of diagnostic work-up. Visit cost is $200 or can be billed to insurance.

Impressions
Appointment for impressions for earmolds, in-ear monitors, custom ear plugs for noise reduction, hunting, shooting, music filtering, swimming, etc. Impressions are $75 per ear.

Hearing Device Check
Appointment for troubleshooting, repair, modification, or adjustments on hearing devices. Cost of visit varies based on what is needed, starting at $100. Not appropriate appointment type if patient needs hearing testing or extensive reprogramming.

No charge consultation to review options for tinnitus treatments.

Hearing Test (Pediatric Patient)
Appointment for hearing test/evaluation on pediatric patient under the age of 18. Cost of visit is $200 or can be billed to insurance.

Repair Pick-Up
Appointment to pick-up repaired hearing devices and ensure programming is correct. Also for hearing devices to be paired to cell phone after repair.

Tinnitus Consultation Only (no hearing testing needed)
60 minute tinnitus consultation includes review of tinnitus history and review of treatment options. Consultation is $250 and cannot be billed to insurance. This visit does not include enough time for diagnostic testing and is most appropriate if you are bringing a recent hearing test (within the past 6 months) to the visit with you.

Lenire Consultation with Device Fitting
Appointment is appropriate for patients wanting to have a consultation for Lenire with same day fitting of the device if deemed a candidate. Cost of consultation is $250.

90 minute tinnitus evaluation with consultation includes review of tinnitus history, full diagnostic testing, and review of treatment options. Consultation is $250 and cannot be billed to insurance. Diagnostic testing can be billed to insurance, if applicable.
